Job Description
TITLE: Youth Transition Program Coordinator
REPORTS TO: Youth Transition Program Manager
PURPOSE: Provide direct Independent Living services to youth with disabilities, ages 14-24 by screening referrals for eligibility for the Youth Transition Program. Prepares youth for living independently and engaging in the community by utilizing the IL model. Run the YT program for the Marysville office and collaborate with Lakewood office when needed.
ESSENTIAL JOB FUNCTIONS

Provide IL Skills Training, Individual Advocacy, Peer Support and Information and Referral. Some specific tasks include: scheduling and conducting initial intakes, conducting IL evaluations, developing IL Goal Plans based on participant’s identified IL goals, assisting the participant to develop an action plan with measurable goals, objectives, and timelines.
All youth participants should be contacted on a monthly basis at a minimum. Having 2-3 appointments a day or serving 7-10 different youth per a week is ideal.
Teach self-advocacy and self-awareness classes in the public high schools. Work directly with students with IEPs/504s and collaborate with teachers, counselors, case managers, and other community partners.
Assist with planning, organizing and implementing Youth Transition Summer Workshop.
Assist with providing supervision and training to practicum students, volunteers and staff who are assisting or working in YT program.
Plan and coordinate monthly youth peer support group meetings/events.
Attend community meetings and provide systemic advocacy for youth with disabilities.
Maintain documentation by developing and maintaining participant files according to established policies.
Participate in weekly consult, monthly staff meetings, trainings, and submit monthly Part B report to ED by the 10th of each month with no or minimum corrections.
Provide effective outreach to youth with various disabilities from diverse educational, economic, and cultural backgrounds. Present information on CFI’s services to other agencies and organizations.
Work with referrals from DVR, DDA, etc. and submit billing on a monthly basis unless other timeframe agreed upon by ED and/or VRC.

QUALIFICATIONS

Bachelor’s degree in Social Work, Psychology, Education or a related field is preferred but not required. Prior personal and professional experience with disabilities may be substituted for education if the individual meets the essential functions of the job.
Experience as a case manager or social worker is a plus.
Working knowledge of Microsoft Office, including but not limited to WORD, EXCEL, Publisher, and PowerPoint.
Ability to become proficient in using CFI’s client data base and any other software programs needed to perform job duties.
Knowledge of ADA regulations is a plus. Being bilingual is a plus.
Must be able to travel occasionally.
Experience living with a disability is a plus.
